John Sorley Memorial Midget Award

This $500.00 bursary is presented annually to a Major Midget Representative Player in the NFMHA. To be considered, the successful candidate must meet the following criteria:

• A Midget Level player;
• Applied to a recognized post-secondary institution;
• Active in the community;
• A positive influence on his teammates; as well as family and friends.

Application process

Please print the application form and submit to the Niagara Falls Minor Hockey Office prior to the deadline. With this application, please attach the following documentation:

• Copy of your most recent report card;
• Proof of application to a recognized Post-Secondary Educational Institution;
• Current resume including, name of employer (if applicable), volunteer work, community and school activities, and sports involvement;
• Three letters of recommendation from non-family members (this could include teachers, coaches, employer, etc.).
